Gateway to Community Living encompasses Alabama Medicaid’s initiative to expand home and community-based resources for Alabamians who are aging or have disabilities, but would prefer to receive services in their own home.
Gateway to Community Living Rebalancing Demonstration
Funded through the Centers for Medicare and Medicaid Services (CMS) Money Follows the Person (MFP) program, Gateway supports individuals who wish to transition from nursing homes and institutional settings to community living. Gateway uses the state’s existing long-term care system as a foundation, allowing individuals currently living in institutional settings to enroll in one of six Home and Community-based Waiver programs.
If you or someone you know is interested in transitioning back to the community setting, please contact the social work department at the long term care facility to initiate a referral. You can also contact the program specialist for Gateway to Community Living by calling 1-800-362-1504.
